Play on a scaled map of the real world: settle your home region, build real cities and compete with nations across continents.
Earth servers replace the normal Minecraft world with a map of the real planet. Survival Earth maps are usually generated at a reduced scale, somewhere between 1:500 and 1:1000, so continents can be crossed in a reasonable time; build projects use the full 1:1 scale. You spawn somewhere on the globe and travel across oceans, mountains and coastlines that match reality.
There are two main styles:
Real terrain shapes the game: popular regions fill up early and capitals tend to end up where you would expect them. When choosing an Earth server, check the map scale, whether resources depend on location, whether nations can declare war and how claims work in crowded areas such as major cities.

Survival Earth servers commonly use scales around 1:500 to 1:1000 so the world can be travelled. Projects that recreate real cities use 1:1 scale, which is enormous and mainly used for creative building.
Usually yes, as long as nobody has claimed the area. On Towny-based Earth servers you found a town at your chosen location; popular regions are often claimed early, so newer servers give you more choice.
